Prior to scheduling an appointment, we will speak briefly over the phone to discuss your particular needs. You can anticipate that I will want to discuss the reason you are seeking treatment, any prior experiences you may have with psychotherapy, and some general demographic information. I may have additional questions based on what we discuss. We will also be sure to address your questions and concerns.

Insurance

I am an out of network provider for all insurance companies. This means that I am not able to bill my services directly to your insurance. I will provide you with a detailed receipt of the services I have provided. If your insurance plan offers out of network benefits, you may be able to use this receipt for partial reimbursement. You will need to check with your carrier to verify if your plan includes out of network benefits. You may also want know if you have a deductible for out of network benefits. 

 

When checking with your carrier, they may ask you which codes will be billed in order to provide you the rate of reimbursement. Below is a list of the most common codes that I may use:

 

  • Initial evaluation: 90792

  • Psychotherapy with medication management: 99213 or 99214 (depending on medication complexity) + 90836

  • Psychotherapy: 90834

Fees and Cancellation Policy

My fee is $400 per 50 min therapy session, and $300 per 30 minute medication management session, unless otherwise discussed.  Payment for each session is expected at time of service and can by made by cash, personal check, or through Zelle. Those submitting for insurance reimbursement are required to pay in full at the time of service

 

Please call as soon as possible if you learn that you must cancel an appointment. A full fee will be charged for any scheduled appointments that you do not attend and for appointments cancelled with less than 24 hours notice. Fees may be subject to change over time. Any fee changes will take effect on January 1st and you will be informed prior to any adjustment.
